What’s in the AON Application Form?

Child or Applicant’s Details
This section gathers the child’s name, date of birth, PPS number, address, and other basic info.

Parent or Guardian Details
You’ll include your own contact details and relationship to the child.

Referral Information
This part asks who is making the referral. It might be you, a GP, a teacher, or another professional.

Statement of Need
Here, you describe your concerns about your child’s development, learning, communication, or other areas. Be as specific and clear as you can. Include examples.

Developmental and Medical History
You’ll be asked about early milestones (walking, talking), any health issues, and previous supports or diagnoses.

Education History
If your child is in preschool or school, this section asks about learning supports and any challenges they’ve experienced.

Other Relevant Information
This is where you can include anything else important, such as family circumstances, strengths, or interests.

Consent
You’ll be asked to sign the form to give permission for the assessment.
